9/4/2023 EnchantaVaughn Approved Behavior: QMHPA held an individual session with Ms. Broadnax. Upon arrival at Ms. Broadnax home, Ms. Broadnax presented with positive affect, evident by her facial expression and body language. QMHPA informed Ms. Broadnax that this session was going to focus on improving reduce depressive symptoms, anxiety and resolve causes leading to normalization of emotional state. Ms. Broadnax did not present any symptoms of SI or HI at the time of the session being conducted. Intervention: QMHPA began the session by assessing Ms. Broadnax emotional well-being. QMHPA questioned Ms. Broadnax about her triggers and stressors. QMHPA explained to Ms. Broadnax different exercises that are useful to help manage her triggers. QMHPA conducted a mental skill building activity that guided Ms. Broadnax with arm/wrist rotations and finger movements. QMHPA taught Ms. Broadnax different techniques to cope with her depression. QMHPA taught Ms. Broadnax about situations that cause her to be depressed. QMHPA encouraged Ms. Broadnax display positive emotions and think positive. QMHPA explained to Ms. Broadnax holding your emotions in will not help you have to let out your feelings at. Response: Ms. Broadnax actively participated in the session. While exploring Ms. Broadnax emotional wellbeing, Ms. Broadnax stated to QMHPA that she was in a positive mood. During the skills building activity, Ms. Broadnax discussed with QMHPA that she has been a little depressed with some things that have been going on in her life; however, she declined to discuss them with QMHPA. Ms. Broadnax discussed with QMHPA that she is at the point where she sometimes just feel like giving in the towel. Ms. Broadnax was able to demonstrate the ability to utilize coping skills when she is upset.
